Former Employees May Request a Copy of Their IRS Form 1095-C
Hastings Insurance is required to provide IRS Form 1095-C to certain employees and covered individuals. Former employees and other covered individuals who did not receive a Form 1095-C may request a copy.
Hastings Insurance will provide Form 1095-C in accordance with applicable federal regulations. Requests must include sufficient information to verify identity and mailing address.
Requests will be fulfilled within 30 days.
